As part of my TV collection, this is season one of Terminator: The Sarah Connor Chronicles. Since season one is only three discs, the cover is for the 3240 x 2175 case. Labels to follow, as well. There are both the 3240 x 2175 and the 3370 x 2175 sizes for season two.
